Is the EMR damaging population health?
The lack of coordination through joined-up systems is exacerbated by our focus on 'the patient record'
Below is a video presentation that I made to a group of healthcare executives last week.

Digitisation of healthcare has taken a wrong turn - largely because we have spent too much time and too many oceans of money digitising the “buff folder” of patient notes and not enough time digitising the systems that coordinate care, which is what we want as patients: joined up care, where all our medical and healthcare professionals are all able to talk to each other and share an action-focused view of care, in real time.
